Authorization and Release

I authorize Kamedis Inc. to use my testimonial, photos, or videos (the “Content”) for marketing, advertising, and promotional purposes in any media, worldwide, without limitation of time.

I understand that:
The Content may be edited or combined with other materials.
I will not receive compensation other than the agreed gift/survey reward.

I confirm that the testimonial reflects my honest opinion and was given voluntarily.
I release Kamedis, its licensees, and successors from any claims related to the use of the Content.
I declare that I am over 18 years old and understand this agreement.
Kamedis has no obligation to use the Content.
